
Many regional veterinary hospitals have our referral forms on hand. Your veterinarian can complete this form. Please feel free to download, print and complete a Patient Referral Form if your pet is being referred to us. Copies are also available at our front desk and by mail.
Please bring this form, copies of your pet's medical records, laboratory test results, radiographs (x-rays), and any medications that your pet is currently taking with you for the appointment.
We recommend not to feed your pet after 10:00pm the evening prior to your pets visit to AVIM, however ask your regular veterinarian for guidance. And, if your pet is a diabetic then do not withhold food or alter his medications prior to a visit with AVIM. Water is fine for your pet to have right up to the time of the visit.
Appointments usually last between 45 and 90 minutes. However, procedures are completed after morning appointments and pets are normally discharged later in the afternoon by arrangement with the attending AVIM doctor. As with all critical care facilities, occassional delays can occur due to emergent situations with the pets of others. Thank you for your patience. We do appreciate your time.
